Bias Incident Report

Providence College is committed to creating and maintaining an environment in which all members of its community are treated with respect and dignity, free from bias and harassment.


Please use this form to report any incidents involving Providence College students, faculty, staff, administrators (whether these behaviors occur on or off-campus), vendors or visitors to the campus. Bias, harassment, and hate crimes include some form of unwelcome conduct that is based on a person’s (or group’s) race, color, national or ethnic origin, sex, gender, sexual orientation, gender identity, religion, disability, age, veteran status, genetic information, or any other applicable legally protected basis.


If there are extenuating circumstances that make anonymity important for a safety or security reason, please make note of that in the report.


If you make an anonymous report using this form, Providence College will not be able to respond directly to you, but will assess the information submitted and consider response options.
